We don’t have listings specifically for this in our directories.
What I’d recommend you do is contact your local humane society and ask them what local programs they have available. Depending on where you live, there may be veterinary clinics that provide such services for disabled an elderly pet owners.
Alternatively, you could clean your dog’s teeth yourself. Here is a link to a video series that shows you how to do that. It is not something we’ve done here, so you’ll want to decide for yourself if this might be a good solution for you.
